Gynecomastia surgery, also known as male breast reduction surgery, is a medical procedure designed to treat the enlargement of breast tissue in males. When the glandular tissue of the male breast swells and becomes overdeveloped, it is called gynecomastia.

The surgery involves techniques such as liposuction to remove excess fat or excision to eliminate glandular tissue and excess skin. Typically performed under anesthesia, recovery times vary but generally allow a return to normal activities within a week.

What is  Gynecomastia?

Gynecomastia is the development of excess glandular breast tissue in males. It is different from pseudogynecomastia, which is enlargement of the chest caused primarily by fat accumulation rather than glandular tissue growth. Many men have a combination of both.

The condition is typically caused by hormonal imbalances between testosterone and estrogen, though other contributing factors include puberty, weight fluctuations, certain medications, steroid use, and natural aging. In most cases, gynecomastia does not resolve on its own once it has stabilized past puberty, and diet and exercise have limited impact on true glandular tissue.

What Causes Gynecomastia?

Understanding the underlying cause matters for determining the right treatment approach. Common causes include:

How Gynecomastia Surgery Works

Traditional Excision

A small incision is made around the edge of the areola (the darker skin surrounding the nipple). Through this incision, the surgeon removes the excess glandular breast tissue directly. This technique is particularly effective for true gynecomastia with firm glandular tissue.

Liposuction

When fatty tissue is also present (pseudogynecomastia or mixed cases), liposuction is used to remove excess fat and contour the chest. This is often combined with excision for comprehensive results.

Skin excision (severe cases)

In cases with significant skin excess or drooping, additional skin removal may be required. Your surgeon will discuss whether this applies to your situation.

Recovery Timeline

Day of surgery

Outpatient procedure. Go home the same day with a trusted companion.

First 48 hours

Rest at home. Some swelling, bruising, and soreness in the chest. Pain medication prescribed.

Days 3 to 10

Soreness improves. Most daily activities can be resumed gently. A compression vest is worn.

1 to 2 weeks

Most patients return to desk work within 7 to 10 days.

3 to 6 weeks

Compression garment worn continuously for 3 weeks, then during the day for 3 more weeks.

Will there be visible scarring?

Scarring from gynecomastia surgery is generally minimal. The periareolar incision is placed along the edge of the areola where it naturally camouflages into the color transition of the skin. Small incision techniques leave even less visible marks. Most patients find the scars fade to near-invisible over 12 to 18 months.

How do I know if I have gynecomastia or just chest fat?

The key difference is the type of tissue involved. Gynecomastia involves firm, rubbery glandular tissue located directly under the nipple and areola. Chest fat is softer and more diffuse across the pec area. Many men have both. A physical examination by an experienced surgeon is the most reliable way to determine which tissues are present and what treatment approach is appropriate.

Can exercise get rid of gynecomastia?

Exercise can help reduce overall body fat, which may improve the appearance of the chest if pseudogynecomastia (fat) is the main issue. However, true gynecomastia involves glandular breast tissue that does not respond to diet or exercise. If the firm tissue under your nipple persists despite being at a healthy weight, surgery is typically the only effective long-term solution.

Does gynecomastia come back after surgery?

In most cases, gynecomastia does not return after surgery. The glandular tissue has been physically removed and will not regenerate. However, if the underlying hormonal cause is not addressed, new tissue growth is theoretically possible, which is why your surgeon will review your medical history and medications carefully before recommending surgery.

Do I need to stop any medications before surgery?

Yes. Your surgeon will review all of your current medications and supplements at your consultation and pre-operative appointment. How long do I need to wear the compression garment?

Will gynecomastia surgery affect my nipple sensation?
